围绝经期妇女子宫切除后现状调查及影响因素分析

THE INVESTIGATION ON SITUATION AND INFLUENCE FACTORS FOR PERIMENOPAUSAL WOMEN AFTER HYSTERECTOMY

目的探讨40-58岁妇女子宫切除后围绝经期的相关症状及影响因素。方法收集在我院就诊的142例子宫良性病变切除后患者的临床资料，对治疗及预后进行相关分析。结果本调查发现干部、工人、文化程度高者，围绝经期综合征的发病率分别比农民、文化程度低者高。Kupperman评分中度和重度的占73．2％。不同的体重指数发生骨量减少和骨质疏松的情况不同。本研究设计时选用倍美力与坤泰胶囊二组作为对照，结果显示坤泰胶囊能显著改善潮热出汗等围绝经期征候群，并随着用药时间的延长，改善情况更加明显，疗效不低于激素组。对骨密度影响二组在治疗前后差异无统计学意义。本研究显示二组乳房疾病和阴道血性物的发生率比较差异无统计学意义，激素组乳腺疾病发生率与治疗前相比有显著性差异。结论围绝经期妇女子宫切除后的健康问题比无子宫切除的有较高发生率。在防治围绝经症状方面坤泰胶囊优于激素组，对于症状严重者、有激素禁忌证者使用坤泰胶囊有很好的效果。

Objective To investigate perimenopausal related symptoms and influencing factors of women aged from 40 to 58 after hysterectomy. Methods Totally 142 cases of post- hysterectomy women for benign lesions from April 2010 to April 2012 in author＇s hospital were collected, the analysis of treatments and prognoses were carried out. Results The survey found that the incidence of perimenopausal syndromes of cadres, workers, people with a high educational level were higher than that of farmers and people with a low educational level. About 73.2% of patients showed a moderate and severe kupperman scoring. Patients who showed different body mass indexes presented different degrees of osteopenia and osteoporosis. In this study, Premarin group and Kuntai Capsule group were selected as control groups. The results showed that Kuntai Capsule could significantly improved perimenopausal syndromes such as hot flash and sweating, the improvement was more obviously with the extension of treatment and the effect was higher than that in Prematin group. No statistical difference was found in the bone mineral density between two groups before and after treatment. This study showed that there was no difference in the incidence of breast diseases and vaginal bloody substances between two guoups. Compared with before treatment, the incidence of breast diseases in hormone group was significantly higher. Conclusions It is show a higher incidence of health prob- lems in perimenopausal women after hysterectomy than those who haven＇t undergone hysterectomy. Kuntai Capsule group is superior to hormone group in the prevention and treatment of perimenopausal symptoms. Kuntai Capsule is a good choice for those who have severe symptoms and contraindications to hormone.
