速度向量成像技术评价妊娠期高血压综合征对胎儿左心室功能影响的研究

Evaluation of fetal left ventricular function in pregnancy—induced hypertension syndrome by velocity vector imaging

摘要 目的探讨速度向量成像技术(VVI)评价妊娠期高血压综合征对胎儿左心室功能影响的临床价值。方法采集并存储65例妊娠期高血压综合征胎儿和65例正常对照组胎儿的动态标准四腔心切面图像,利用VVI软件测量左心室心肌长轴方向运动指标。结果与对照组相比,24+0周后妊娠期高血压综合征组收缩期峰值应变(S)、应变率(SRs)和舒张期应变率(SRd)均明显减低(P<0.05),28+0周后妊娠期高血压综合征组收缩期峰值速度(Vs)、舒张期峰值速度(Vd)均明显减低(P<0.05)。妊娠期高血压综合征组Vs、Vd均随孕周的增加而增大(P<0.05),而S、SRs和SRd随孕周增加差异无明显变化(P>0.05)。结论妊娠期高血压综合征胎儿心功能在孕期已发生改变;VVI技术可敏感地检测出其左心室功能的变化。 Objective To investigate the clinical value of velocity vector imaging(VVI) in assessment of fetal cardiac function of pregnancy-induced hypertension syndrome. Methods The study included 65 fetuses of pregnancy-induced hypertension syndrome,and 65 fetuses of normal pregnancy. Digital dynamic four-chamber images were collected, then the left ventricular longitudinal indexes were measured. Results Compared with the control group, the systolic maximum rate(S), systolic maximum strain rate(SRs), and diastolic maximum strain rate(SRd) in left ventricle of fetuses with pregnancy-induced hypertension syndrome after 24^+0 week and systolic maximum velocity(Vs) and diastolic maximum velocity(Vd) after 28^+0 week were obviously lower than the corresponding parameters of normal fetuses(P〈0.05). Conclusion The ventricular function of fetal left heart of pregnancy-induced hypertension syndrome was damaged. VVI is a sensitive technique to reflect the variation of fetal left ventricular function.
