原发性肝癌患者分级护理方案的实施效果

Clinical Study of Grading Nursing in Patients with Primary Liver Cancer

目的建立肝癌患者分级护理要素指标,形成肝癌患者的分级护理方案,并探讨其临床实施效果。方法采用Delphi专家函询法对肝癌患者分级护理指标体系的结构和内容进行筛选和修改;采用方便抽样和类实验研究的方法将北京市某肝病专科医院2010-2011年住院的240例原发性肝癌患者分为研究组和对照组,研究组采用肝癌患者分级护理方案(在Delphi专家函询法的基础上建立肝癌等级护理分级指标体系,并结合临床工作经验和临床条件,制定肝癌患者分级护理方案),对照组采用传统护理方法和护理措施,比较两组患者的满意率、护患纠纷和并发症发生情况。结果肝癌患者等级护理分级指标体系包含4个一级指标和28个二级指标。研究组患者满意率高于对照组,护患纠纷发生情况及部分并发症的发生率低于对照组,差异均有统计学意义(均P<0.05)。结论肝癌患者分级护理方案更加符合肝癌患者的疾病特点,有助于提高护理质量、减少医疗纠纷,可为肝癌临床护理工作提供参考。

Objective To establish the grading nursing index and form the detailed nursing plan of grading nursing in patients with primary liver cancer.Methods Delphi procedure were used to establish the essential indicators system and filter and revise the content.By convenience sampling,a total of 240patients with primary liver cancer were selected and divided into experimental group and control group.The experimental group was given the new plan of grading nursing,while the control group was given the routine grading nursing care.The patient＇s satisfaction rate, conflict between patient and nurses and incidences of complications were compared between two groups.Results The grading nursing indicators system included 4indicators and 28sub-indicators,the satisfaction rate in experimental group was higher than which in contrl group,and the incidence of patient-nurse conflict and complications were less than control group（P〈0.05）.Conclusion The new nursing plan of grading nursing was more accorded with the feature of the disease,and it can improve the nursing quality,reduce medical disputes and it provide evidences for clinical nursing of patients with primary liver cancer.
