急性心肌梗死患者早期活动评估指标的研究

Indicators for assessment of early exercise risk in patients of acute myocardial infarction

目的 确定急性心肌梗死（AMI）患者早期活动评估指标,为制定个体化活动及护理方案提供依据.方法 建立AMI患者早期活动评估指标及分级草案;用德尔菲法对23名专家进行2轮调查.结果 2轮咨询专家的积极系数分别为92％、100％,权威系数均为0.94,协调系数为0.23、0.35.经2轮咨询,各项指标变异系数为13％～19％,指标分级满分比87％～100％,确立了用于AMI患者早期活动方式评定的10项指标和各指标的分级.结论 AMI患者早期活动评估指标有较好的代表性和可信度,后续研究将对指标分级赋值,从而用风险积分指导制定AMI患者早期活动方式.

Objective To determine indicators for assessment of early exercise risk in patients of acute myocardial infarction,and to provide references for formulating individualized exercise and nursing care programs.Methods A draft of indicators and their ratings for assessment of early exercise risk was created,then 23 experts were asked to participate in a two-round Delphi survey.Results The response rate was 92% in round 1 and 100% in round 2.The authority coefficient was 0.94 in both rounds and the Kendall＇s coefficient of concordance was 0.23 in round 1 and 0.35 in round 2.The coefficient of variation for each indicator ranged from 13% to 19% and the ratio of full score for ratings of each indicator ranged from 87% to 100% in the second round.Then we developed 10 indicators and their ratings for early exercise risk assessment after acute myocardial infarction.Conclusion The indicators for assessment of early exercise risk seem to be reliable and representative.Scoring of each indicator is to be studied in the future research,thus to calculate risk score for formulating early exercise program for patients suffering from acute myocardial infarction.
