XS-1000i血细胞分析仪白细胞警示信息镜检形态学分析

Analysis of the microscopic examination results of leukocyte alarm information from XS-1000i hematology analyzer

目的比较分析Sysmex XS-1000i血细胞分析仪白细胞警示信息与手工镜检的符合程度。方法选取出现白细胞警示信息的标本1500份,进行手工染色显微镜检查,观察血涂片细胞形态,与仪器报警信息进行对比分析。结果 1500例白细胞报警标本中,出现警示信息1652次,手工镜检真阳性960次,阳性预测值58.1%。原始细胞、未成熟粒细胞、异形淋巴细胞、核左移和散射图异常五项报警信息的真阳性率分别是50.0%、57.9%、22.4%、100.0%和92.6%。95例散射图异常标本除7例白细胞数量及形态结构无变化外,其余皆有白细胞或红细胞血小板数量形态的异常变化。结论 Sysmex XS-1000i血细胞分析仪能有效筛检出白细胞系统出现异常的标本;仪器出现警示信息应进行手工显微镜复检。

Objective To compare and analyze the coincidence degree of the microscopic examination and the alarm information of leukocyte from XS- 1000i hematology analyzer. Methods A total of 1500 samples with leucocyte alarm information were selected for staining microscopy to observe the cellular morphology on blood smear,then the results were analyzed and compared with alarm information from XS- 1000i hematology analyzer. Results In the 1500 samples,alarm information were found 1652 times,and 960 cases were confirmed true positive by manual microscopy. The positive predictive value was 58. 1%. The true positive rates of 5 pieces of alarm information including the primitive cells,immature granulocytes,atypical lymphocytes,left shift and abnormal scatter plot were 50. 0%,57. 9%,22. 4%,100. 0% and 92. 6%,respectively. In 95 specimens with abnormal scatter plot,7 cases were found no changes in quantity and morphological structure of white blood cells,while the rest were all found abnormal changes in quantity and morphology of white blood cells or red blood cells. Conclusion Sysmex XS-1000i hematology analyzer system can effectively screen out specimens with abnormal white blood cells; When instrument sound warning message,manual microscopy should be carried out for recheck.
