吞咽评估与口腔护理在中风后吸入性肺炎患者中的应用

Dysphagia assessment and intensified oral care reduce pneumonia after stroke

目的：探讨评估早期中风患者吞咽评估与加强口腔护理对降低中风后吸入性肺炎发生的影响效果。方法：选择2011年10月～2013年5月我院中风患者106例随机等分为观察组和对照组。观察组采用Gugging吞咽功能量表评估,评分〈15分者采取口腔护理。对照组患者给予常规护理。在30 d后观察患者吸入性肺炎发生率、住院时间、鼻胃管饮食患者例数。结果：观察组患者吸入性肺炎发生率低于对照组（P〈0.05）,住院时间短于对照组,鼻胃管饮食患者例数也少于对照组,P〈0.05。结论：中风后早期Gugging吞咽功能困难评估与加强口腔护理可有效减少吸入性肺炎的发生。

Objective： To investigate whether the aspiration pneumonia incidence could be reduced by an early assessement for dysphagia and intensified oral care. Methods： 106 patients with acute stroke were divided into control group and observation group. The observation group patients were assessed with the Gugging swallowing assessment for dysphagia,and intensified oral care with the score less than 15. The patients in the control group were assessed by convetional ways. To Compare aspiration pneumonia incidence,mortality,and length of stay in hospital and nasogastric between two groups ofter 30 days. Results： The incidence of pneumonia in obseration group was lower than the control group（ P〈0. 05）,in hospital time was shorter than the control group（ P〈0. 05）. Conclusion： Early dysphagia assessement by the Gugging swallowing assessement and intensified oral care can effectively reduce the incidence of pneumonia.
