腰大池持续引流术在基层医院神经外科疾病治疗中的应用

Clinical application of continuous lumber cistern drainage in neurosurgery disease at the basichospital

目的探讨腰大池引流术在基层神经外科疾病治疗中的应用。方法采用微创颅脑引流套NSYL-ⅡF5L,于腰3-4或腰4-5椎间隙将腰大池引流管置入蛛网膜下腔内,末端接连腰大池引流瓶,组成封闭的引流系统。结果将该技术应用于49例蛛网膜下腔出血、外伤性脑膨出、脑积水、难治性脑脊液漏以及颅内感染患者的治疗中,均取得了良好的疗效。结论腰大池持续引流通过缓慢排放脑脊液,能持续保持理想的颅内压,营造良好的颅内环境,改善脑血流灌注,减轻病灶区脑组织的缺氧。其具有操作简单、创伤小、疗效确切、并发症少等特点,适合在基层医院神经外科临床治疗中广泛开展。

[ Objective ] To investigate application of continuous lumber drainage in neurosurgery disease at the basic unit hospital. [ Methods ] Use the minimal invasive brain drainage sleeve NSYL- I1 FSL, 3-4 or 4-5 in the lumbar spinal vertebra clearance with lumbar cistern drainage tube into the subarachnoid space, the terminal con- nects the waist cisterna magna drainage bottle, composite seal drainage system. [Results] The technology applied achieved good curative effect in 49 cases of patients treatment included subarachnoid hemorrhage, traumatic en- cephalocele, hydrocephalus, refractory cerebrospinal fluid leakage, intracranial infection. [ Conclusions ] The cere- brospinal fluid slow emissions by continuous lumber drainage, intracrania[ wiU maintain an ideal pressure, to create a good intracranial environment, improve cerebral blood flow perfusion, hypoxic cerebral tissue lesions. It has the characteristics of simple operation, small trauma, definite curative effect, less complications, suitable for the exten- sive development in the clinical treatment of department of neurosurgery in basic hospital.
