功能神经导航联合术中皮层电刺激在功能区肿瘤中的应用 被引量:1

摘要 目的:探讨弥散张量示踪成像( DTT)融合神经导航联合术中皮层电刺激技术在手术治疗颅内运动区肿瘤中的临床应用价值。方法对31例运动区肿瘤患者(其中胶质瘤19例,脑膜瘤8例,海绵状血管瘤3例,脑脓肿1例)。术前行MRI薄层扫描,DTT,并在导航仪上进行影像融合,了解肿瘤及锥体束相对关系,设计手术入路。术中在导航引导下定位肿瘤,锥体束;术中皮层电刺激定位功能区并实时定位肿瘤周围脑功能边界,显微镜下切除肿瘤。结果胶质瘤中全切除15例,次全切除3例,大部分切除1例,脑膜瘤及海绵状血管瘤均全切除。术后5例出现一过性肌力下降,术后1个月肌力恢复。其余术后与术前相比肌力有所提高或基本相似。术后3~6个月随访,所有患者肌力水平与术前比较均有好转或不变。结论磁共振、纤维示踪融合导航,结合皮层电刺激技术,可实现最大程度切除运动区肿瘤的同时最大限度的保护脑功能,减少术后并发症的发生。
