APACHEⅡ评分在预测急诊危重病人预后中的应用价值

The value of the application of APACHE Ⅱ score in predicting the prognosis of emergency critically ill patients

目的为了探讨APACHEⅡ评分在预测急诊危重病人预后中的应用价值,从而为临床急诊危重病人预后的预测实际提供指导和依据。方法本文选取了我院2009年1月～2011年12月期间收治的急诊危重患者211例为研究对象,观察比较不同APACHEⅡ评分段患者的死亡发生率,同时比较存活患者与病死患者的平均年龄、ICU住院时间、性分布情况以及APACHEⅡ评分结果。结果随着患者APACHEⅡ评分水平的不断升高,实际病死率、预计病死率以及阳性率均显著的升高；在平均年龄上,组间比较,死亡组患者的平均年龄显著的高于存活组患者的平均年龄；在APACHEⅡ评分上,组间比较,死亡组患者的APACHEⅡ评分显著的高于存活组患者的APACHEⅡ评分。结论在临床实践过程中,APACHEⅡ评分可以准确的评价患者的病情危重程度,同时也可以有效的预测急诊危重病人的预后。

Objective To investigate the value of the application of APACHE Ⅱ score in predicting the prognosis of emergency critically ill patients. Methods 211 cases of emergency critically ill patients in our hospital from January 2009 to December 2011 were chosen as the research object. The incidence of death for different APACHE Ⅱ scores of patients was compared, and the mean age of survivors and dead patients, and ICU days of stay were compared. Results With the rising of APACHE Ⅱ score levels, the actual and expected mortality were significantly increased, and the differences were statistical significant（P〈0.05）. The average age of death of patients was significantly higher than that of patients survived（P〈0.05）. The APACHE Ⅱ score in the death group was significantly higher than that of the survival group（P〈0.05）.Conclusion In clinical practic, APACHE Ⅱ score can accurately assess the patient＇s severity of illness, and also can effectively predict the prognosis of emergency critically illpatients.
