应用Transbuccal Trochar微创治疗单纯下颌骨角部骨折的临床观察 被引量:4

摘要 目的:观察应用transbuccal trochar器材经口内入路坚固内固定,治疗下颌角部骨折的临床疗效。方法:2010-2012年间,下颌骨角部骨折患者25例,应用transbuccal trochar器材经口内入路复位内固定13例,另外12例按照传统口外入路行手术切开复位内固定,对两组患者的临床疗效加以对比。结果:口内组及口外组均达到良好的疗效,包括良好的骨折断端对位、正常的张口度及原有咬合关系的恢复等。口内组有1例出现了创口感染,口外组有3例术后出现暂时性面瘫;与口外组相比,口内组避免了面部瘢痕并减少了手术创伤。结论:与口外入路相比,经口内入路应用transbuccal trochar治疗下颌角部骨折,手术时间短,术后并发症少,兼顾了患者对美观和功能的双重要求。 Objective: To observe the clinical efficacy of transbuccal trochar in the minimal invasive reduction of mandibular fractures. Methods:A total of 25 cases of mandibular angle fractures were reviewed. 13 cases were treated by intraoral incision with transbuccal mini incision, and the other 12 cases were treated by traditional extra-oral approach. The clinical outcomes of the two groups were compared. Results: It had been found that both techniques fulfill the functional requirements of the patients. One patient treated by intraoral approach suffered wound infection, and 3 cases treated by extra-oral approach resulted in facial paralyses. Patients treated with submandibular approach developed obvious sightly scars, whereas transbuccal approach results in minimal scarring. Conclusion: The transbuccal approach gives promising results in terms of function and esthetics in the reduction of mandibular angle fracture.
