肝内胆管细胞癌的超声造影表现分析

Performance of intrahepatic cholangiocarcinoma on contrast-enhanced ultrasound

目的探讨肝内胆管细胞癌(ICC)超声造影的表现特点。方法回顾性分析32例经穿刺或手术病理证实为ICC患者的临床、常规超声图像及超声造影动态资料。结果 32例ICC患者共34个病灶。超声造影在时间上的分布特点均表现为"快进快出"增强方式;空间上的分布特点有3种类型:20个(58.8%,20/34)周边环状高增强向内充填型,5个(14.7%,5/34)整体均匀增强型,9个(26.5%,9/34)周边环状高增强不向内充填型。结论 ICC在超声造影上具有特定的增强方式,超声造影有助于鉴别诊断。

Objective To evaluate the value of contrast-enhanced ultrasound （CEUS） in the diagnosis of intrahepatic cholangiocarcinoma （ICC）. Methods The clinical data and routine ultrasound and contrast-enhanced ultrasound imagings of 32 ICC cases confirmed pathologically were analyzed retrospectively. Results All lesions demonstrated hyperenhancement in the arterial phase, and hypoenhancement in the portal vein phase or delayed phase,with the pattern of‘fast in and fast out’. Three kinds of enhancement patterns were detected： 58.8% （20/34） with peripheral rim-like enhancement and concentric filling, 14.7% （5/34） with whole enhancement, and 26.5% （9/34） with peripheral rim-like enhancement and non-concentric filling. Conclusion ICC has typical CEUS appearance and CEUS plays a useful role in its differentional diagnosis.
