摘要
目的探讨心力衰竭合并心房颤动(房颤)患者血浆B型钠尿肽(BNP)水平及其与患者心功能指标及分级的相关性。方法将2012年5月至2013年5月平顶山市第二人民医院院收治的经超声心动图确诊为心力衰竭的78例患者分为房颤组(38例)和心律失常组(40例),另选取40例正常体检者为对照组,对比分析三组受试者BNP水平及其与心功能指标及分级的相关性。结果与对照组相比,心力衰竭患者BNP、收缩末容量(ESV)、舒张末期容积(EDV)显著上升,而心脏射血分数(EF)水平显著下降,其中房颤组患者BNP、ESV、EDV水平显著高于心律失常组,而EF水平则显著低于心律失常组(P<0.05)。心力衰竭合并房颤组患者随着心力衰竭程度的加深,ESV、EDV水平显著上升,而EF水平显著下降(P<0.05)。经Pearson相关性分析可知,BNP与ESV、EDV成正相关,而与EF成负相关(P<0.05)。结论心力衰竭合并房颤患者的血浆BNP水平显著高于合并心律失常的患者,随着心力衰竭程度的加深,其水平显著增加,且血浆BNP水平与患者心功能具有显著相关性。
Objective To investigate the levels of B-type natriuretic peptide (BNP) and the classification relevance for cardiac function of patients with heart failure and atrial fibrillation. Methods A total of 78 cases of echocardiography diagnosed heart failure were divided into atrial fibrillation group (38 eases) and arrhythmia group ( 40 cases) from May 2012 to May 2013 in Pingdingshan City Second People's Hospital hospital. The other 40 .normal subjects were selected as the control group. The levels of BNP and its correlation with cardiac function and classification of the three groups were analyzed and compared. Results The BNP, end-systolic capacity(ESV), end-diastolic volume(EDV) were significantly increased than control group, while the levels of ejection fraction (EF) was significantly decreased, BNP, ESV and EDV level of the atrial fibrillation group were significantly higher than the arrhythmia group, while EF level was statistically signifi- cantly lower( P 〈 0.05 ). ESV, EDV levels increased significantly along with the seriousness increase of heart failure in patients with heart failure combined with atrial fibrillation, while the EF level significantly decreased( P 〈0.05 ). By Pearson correlation analysis, BNP was proportional with ESV, EDV but inversely proportional with EF ( P 〈 O. 05 ). Conclusion Plasma BNP levels of Heart failure patients with atrial fibrillation was significantly higher than heart failure patients with arrhythmia, and the levels were significantly increased along with the seriousness increase of heart failure, furthermore, plasma BNP levels and cardiac function are significantly correlated.
