超声对孕晚期子宫下段宫壁厚度的观察与分析 被引量:2

摘要 目的探讨产前利用超声测量子宫下段宫壁厚度预测瘢痕子宫破裂的可行性。方法超声检查并用谐波成像技术对196例既往有剖宫产史的孕妇,在妊娠晚期对子宫下段前壁肌层厚度进行连续多次检测,并与分娩及手术的结果相对照。结果有3例(1.5%)孕妇存在子宫不完全破裂。未发现子宫完全破裂者。存在子宫不完全破裂的孕妇子宫下段肌层厚度明显小于未存在子宫破裂的孕妇。子宫下段前壁肌层厚度=2.5mm为预测子宫破裂的最佳临界值。其灵敏度为90.0%,特异度为97.0%。结论应用超声检测妊娠晚期子宫下段前壁肌层厚度的意义重大,子宫下段前壁肌层厚度<2.5mm可预测瘢痕子宫破裂。
