C-反应蛋白在小儿细菌性肺炎和支原体肺炎中的表达及临床意义

Clinical significance of detection of C-reaction protein in diagnosis of pediatric bacterial pneumonia and mycoplasma pneumonia

目的探讨C-反应蛋白(C R P)在小儿细菌性肺炎和支原体肺炎中的表达及临床意义。方法随机选取2012年10月—2013年7月我院收治的细菌性肺炎患儿36例,支原体肺炎患儿32例,分别检测入院后第2天、治疗第4天、出院时C R P的变化。结果随着病情的好转,2组患儿的C R P值及阳性率均逐渐下降到正常水平。细菌性肺炎组在入院时、治疗第4天,C R P值及阳性率均高于支原体肺炎组,差异有统计学意义(P<0.01);出院当天2组患儿C R P值基本恢复正常,差异无统计学意义(P>0.05)。结论 C R P在小儿细菌性肺炎初期即明显升高,且随着病情的好转逐渐下降到正常水平,而支原体肺炎在病情较重或并发感染时才会升高。C R P是两种肺炎鉴别诊断的有效指标。

Objective To study the expression and clinical significance of detection of C-reaction protein in diagnosis of pediatric bacterial pneumonia and mycoplasma pneumonia. Methods 36 cases of bacterial pneumonia and 32 cases of mycoplasma pneumonia were randomly selected from October 2010 to July 2013 in our hospital. The change of CRP level was determined separately on the first two days after admission, 4 days after treatment and at discharge. Results With the improvement of the condition, CRP level and positive rate of the two groups were decreased to normal level.CRP level and positive rate of bacterial pneumonia group were higher than mycoplasma pneumonia group on admission and 4 days after treatment,and the difference was statistically significant （P〈0.01）.CRP level of two groups were decreased to normal levels at discharge, and the difference have no statistical significance （P〉0.05）.Conclusion CRP is significantly higher in the early bacterial pneumonia, moreover, with the improvement of the condition gradually decreased to normal levels. However, CRP will rise in serious condition or complicated infection in mycoplasma pneumonia.CRP is an effective indicator of the differential diagnosis of two kinds of pneumonia, worthy of promotion.
