乳晕切口治疗乳腺良性肿块38例临床观察

Clinical observation of 38 cases of benign breast tumor using periareolar incision

目的观察不同切口手术方式治疗乳腺良性肿块的临床效果。方法以72例乳腺良性肿块患者为研究对象,分别采用传统手术切口和乳晕切口进行治疗,观察2组的疗效及患者满意度。结果 2组手术时间、并发症比较差异无显著性（P〉0.05）。乳晕切口组对手术切口瘢痕感到满意者32例,传统手术组对手术切口瘢痕感到满意者17例,2组比较差异有显著性（P〈0.01）。对72例患者随访1年～3年,共有32例患者术后生育,2组患者不能哺乳者所占比率大体相当,差异无显著性（P〉0.05）。结论采用乳晕环形切口治疗乳腺良性肿块,疗效与传统切口手术相当,但是瘢痕更加隐蔽,患者满意度高,临床实践中须严格掌握适应证和禁忌证,防止并发症的发生。

Objective To explore the clinical effect of different incision operation mode and patient satisfaction for treatment of benign breast tumor. Methods 72 cases of patients with benign breast tumor as the research object were treated with traditional operation incision and periareolar incision respectively, the curative effect and patient＇s satisfaction of two groups was observed. Results The difference of operation time, complications between two groups was not significant （P〉0.05）. The operation incision scar satisfaction of areola incision group reached 84.2% （32/38）, while it was 50% （17/34） in the traditional operation group, the difference was significant （P〈0.01）. 72 patients were followed up for 1～3 years, a total of 32 patients in the two groups were birth, unbreastfeeding rate was roughly same, the difference was not significant （P〉0.05）. Conclusion Using the periareolar incision in the treatment of benign breast lumps, the efficacy was as good as conventional incision operation, however, the scar was more covert with high patient satisfaction, but it should strictly grasp the indications and contraindications on the clinical practice to prevent complications.
