慢性阻塞性肺疾病住院患者抑郁现状及其影响因素研究

Depression of Hospitalized Patients with Chronic Obstructive Pulmonary Disease and Its Influencing Factors

目的明确慢性阻塞性肺疾病住院患者合并抑郁的现状,探索影响患者发生抑郁的因素。方法采用抑郁自评量表对164例慢性阻塞性肺疾病住院患者进行测评,并采用χ2检验和二元Logistic回归分析探索患者合并抑郁的影响因素。结果 164例患者中,存在抑郁76例,占46.3%,患者的抑郁得分高于中国常模,差异具有统计学意义(P<0.001);二元Logistic回归分析显示:病情、病程、文化程度、自评经济压力和自评健康状况5个变量对慢性阻塞性肺疾病患者抑郁的影响具有统计学意义(P<0.05)。结论慢性阻塞性肺疾病患者的抑郁情况比较严重,且病情、病程、文化程度、自评经济压力和自评健康状况5个因素均是影响患者抑郁的主要因素。

Objective To explore the present situation of depression and its influencing factors among hospitalized patients with chronic obstructive pulmonary disease（COPD）. Methods A questionnaire survey was conducted among hospitalized patients with COPD with Zung Self-rating Depression Scale. χ2test and binary logistic regression analysis were used to explore the influencing factors of depression among the patients. Results There were 76 patients suffering depression among 164 COPD patients, which took a proportion of 46.3%, and depression score was higher than that of the Chinese norm（P〈0.001）. Binary logistic regression analysis showed that state of illness, course of illness, education level, self-evaluated economic pressure and self-evaluated health status significantly affected the prevalence of depression among COPD patients（P〈0.05）. Conclusion The depression of COPD patients remains severe, and state of illness, course of illness, education level, economic pressure and self-evaluated health status are the main influencing factors of depression among the patients.
