盆底器官脱垂患者肛提肌的动态MRI研究 被引量:2

摘要 目的探讨盆底器官脱垂患者肛提肌的动态MRI表现特征,建立基于MRI的盆底肌肉和韧带损伤的诊断技术。方法收集临床确诊的盆底器官脱垂患者32例,对照组15例。所有病例术前均接受盆底动态MRI检查,分别获得静息状态下和用力排便(施加最大腹压)时的轴位、矢状位和冠状位的动态MRI影像资料,并采用医学图像分析软件对获得的原始图像进行对比观察和二维测量。结果盆底器官脱垂患者肛提肌明显变薄,肌肉连续性中断,对抗腹压能力下降致器官移位和形态失常。结论 MRI检查可以很好地观察盆底肌肉、筋膜和韧带,动态检查还可以评价功能状态。将动态MRI检查应用于盆底器官脱垂患者可以更好地了解盆底解剖结构损伤和功能变化,指导临床,协助制定完善的手术方案。 Objective Probe to study the dynamic MR imaging characteristic of levator ani muscle in pelvic organs prolapse (POP) patients, and to establish a set of evaluation system with the use of dynamic MR Imaging for the diagnosis and evaluation of POP. Methods 32 POP patients and 15 controls contrast who have had operation in the Fuzhou General Hospital who were finally diagnosed dinically were enrolled in our study. All participants underwent MRI scans of the pelvis at static and at Valsalva. Measuring was performed in coronal, sagittal and axial images using the medical imaging software. Results The levator ani muscle is thinner and incontinuit in POP group. Conclusion MR imaging permits this detailed study. Static images show the morphology. Dynamic images show the functional changes. It is helpful in making surgery project.
