The radiation dose investigation of coronary artery CT angiography

摘要 目的分析双源CT冠状动脉CTA扫描资料,调查目前双源CT冠状动脉CTA的辐射剂量,为临床工作中病人的个性化扫描提供参考方案。方法回顾性分析2011年1月至2013年1月于南方医院行冠状动脉CTA检查的1313例患者的男女构成比、平均心率(分为低心率(≤65bpm)、中等心率(66~79bpm)、高心率(≥80bpm))、心率变化值[正常(0~3)、中度(4~10)、重度(〉10)]、有效剂量,了解其与有效剂量的关系。结果1313例患者中,其中男性766例,女性547例,男/女比例约为1.4:1。低心率337人,有效剂量为(14.66±2.62)mSv,中等心率502人,有效剂量为(12.41±2.34)mSv,高心率474人,有效剂量为(10.65±2.96)mSv。心率波动正常244人,有效剂量为(11.21±2.23)mSv,心率波动范围中度777人,有效剂量为(12.03±2.68)mSv,心率波动范围重度292人,有效剂量为(14.14±3.83)mSv。结论双源CT冠状动脉CTA平均心率越低,有效剂量越高。心率波动值越大,有效剂量越高,在实际工作中应采用合适的扫描方案,使得病人在扫描过程中所受的辐射剂量较低。 Objective To investigate the radiation dose of coronary artery CT angiography through analyzing the epidemiologic data of the dual source CT thus providing a solution for individual scanning. Methods A total of 1313 consecutive patients undergoing CTCA at Nan Fang hospital were retrospectively analyzed, the constituent ratio of the gender, mean heart rate(divided in three groups: low, intermediate, and high ( ≤ 65, 66 - 79, and ≥ 80 beats/min) respectively), heart rate variability (normal: 0 - 3, moderate: 4 10, and severe: 〉10) and effective dose were retrospectively summarized. Results There were 766 men and 547 women; with a male predilection of 1.4 to 1, group 1 : HR ≤ 65 bpm (n = 337, ED = 14.66 ± 2.62 mSv), group 2: HR ≥ 66 bpm to ≤ 79 bpm (n = 502, ED= 12.41 ± 2.34 mSv), group 3: HR ≥ 80 bpm (n = 474, ED = 10.65 ± 2.96 mSv). All patients were divided into three groups according to the heart rate variability, group 1: normal (n = 244, ED = ll.21 ± 2.23 mSv), group 2: moderate (n = 777, ED = 12.03 ± 2.68 mSv), group 3: severe (n = 292, ED = 14.14 ± 3.83 mSv). Conclusion The faster the mean heart rate beats, the more the effective dose. The more the heart rate fluctuates, the more the effective dose. Appropriate protocols should be adopted in practice to reduce the radiation dose.
