3.0T磁共振扩散加权成像在胰腺内分泌肿瘤的应用研究

摘要 目的探讨磁共振扩散加权成像(DWI)在胰腺内分泌肿瘤中的应用价值。方法收集病理证实的7例胰腺内分泌肿瘤患者影像资料,均经3.0 T磁共振常规及DWI扫描,分别测量病灶和正常胰腺组织的表观弥散系数(ADC)值,并应用SPSS 17.0软件选用Mann-Whitney U检验进行比较。结果 7例胰腺内分泌肿瘤中,4例位于胰颈,1例位于胰头,1例位于体尾部,1例位于胰体。所有病灶均呈稍长、长T1及T2信号,DWI序列呈高信号,病灶的ADC值为(1.164±0.299)×10-3mm2/s,明显低于正常胰腺组织的ADC值为(1.462±0.251)×10-3mm2/s,并差异有统计学意义(U=8.0,P<0.05)。结论在内分泌肿瘤的检测中,DWI可对常规MR扫描起到很好的辅助诊断作用,同时能进行定量分析。 Objective To investigate the value of diffusion-weighted MRI of pancreatic neuroendocrine tumors at 3.0 T MR. Methods 7 patients with pathologically proved pancreatic neuroendocrine tumor (NET) underwent T1, T2 and DWI-MR at a 3.0 T scanner. Then apparent diffusion coefficients (ADC) in the pancreatic tumor and normal pancreatic tissues were measured respectively. All parameters of different tissues were analyzed and compared by SPSS 17.0. Results Among 7 cases of pancreatic neuroendocrine tumors , 4 were in the neck of pancreace, 1 in head, 1 in body and tail, and 1 in body.The ADC value of all pancreatic lesion was ( 1.164 ± 0.299) ×10-3mm2/s, significantly lower than that of normal pancreatic tissue ( 1.462±0.251 ) × 10 3mm2/s (U=8.0, P〈 0.05). Conclusion DW MRI is a valuable adjunct to T1w, T2w imaging in pancreatic NET detection.Also it can quantitatively differentiat NET from normal pancreatic tissue with ADC measurements.
