CD64指数、血清降钙素原、IL-6及TNF-α鉴别细菌感染的临床价值 被引量:16

摘要 目的探讨外周血中性粒细胞表面CD64指数、降钙素原(PCT)、白细胞介素6(IL-6)及肿瘤坏死因子α(TNF-α)在细菌感染中的诊断价值。方法将82例细菌感染患者作为感染组,32例健康体检者作为对照组。CD64指数采用EPICS-XL型流式细胞仪检测,PCT、IL-6及TNF-α采用双抗体夹心酶联免疫吸附测定(ELISA)检测,微生物培养检测采用MicroScan AS-4微生物分析仪。通过受试者工作特征(ROC)曲线评价上述4项指标在鉴别诊断中的应用价值,并进行相关性分析。结果感染组患者外周血中性粒细胞表面CD64指数、PCT、IL-6、TNF-α水平均高于对照组,差异有统计学意义(P<0.05)。CD64指数、PCT、IL-6、TNF-α单独检测及其联合检测的ROC曲线下面积分别为0.951、0.864、80.824、0.907及0.969,上述4项指标在细菌感染中的诊断率分别为86.8%、80.7%、74.6%及83.3%,4项指标联合应用在细菌感染中诊断率为93.0%。细菌感染患者血清CD64指数分别与PCT、IL-6、TNF-α呈正相关。结论血清CD64指数、PCT、IL-6及TNF-α联合检测对细菌感染的鉴别诊断有一定的诊断价值。
