开腹与腹腔镜胆囊切除术疗效比较分析

Comparative analysis of effect of open and laparoscopic cholecystectomy

目的:分析腹腔镜胆囊切除术与开腹胆囊切除术治疗胆囊结石的临床效果。方法:收集158例胆囊结石患者的临床资料,依手术方式分为两组,采用腹腔镜胆囊切除78例为腹腔镜组,采用开腹胆囊切除80例为开腹组,分析比较两种手术方法的临床疗效。结果:腹腔镜组的术中出血量、术后并发症明显少于开腹组,腹腔镜组手术时间、术后肛门排气时间、住院时间明显短于开腹组。两组比较,差异有统计学意义(P<0.05)。结论:腹腔镜组疗效较开腹组好,是治疗胆囊结石安全、有效的手术方式。熟练掌握腹腔镜操作技术,掌握好适应证及禁忌证,腹腔镜胆囊切除术适合在基层医院开展和推广。

Objective To analysis of laparoscopic cholecystectomy and open cholecystectomy clinical effect of the treatment of cholelithiasis. Methods Collected the clinical data of 158 patients with cholelithiasis, according to the operation method is divided into two groups,use of laparoscopic cholecystectomy for 78 cases of laparoscopic group,the open cholecystectomy group in 80 patients in laparotomy group,comparing the clinical curative effect of two kinds of operation methods. Results The amount of bleeding and postoperative complications of laparoscopic group was obviously less than laparotomy group,group of laparoscopic operation time, postoperative anal exhaust time, hospital stay is much shorter than laparotomy group.The comparison between the two groups, the difference was statistically significant（P〈 0.05）. Conclusion The effect of laparoscopic group better than laparotomy group,it is safe and effective operation method in the treatment of cholelithiasis. Skilled laparoscopic operation technique, to master the indications and contraindications, laparoscopic cholecystectomy can be carried out and promotion in basic hospitals.
