摘要
针对医疗成本居高不下和医疗服务资源分配研究限于对某单一医疗服务资源的分配问题,提出了一种基于疾病诊断相关组(DRGs)和模糊需求的医疗服务资源整体层面的分配优化模型。在对以往基于DRGs的医院资源分配研究回顾和对可信性理论的模糊需求介绍的基础上,利用该医疗服务资源分配优化模型在既定需求满足率下对某医院单纯性阑尾炎病种季度资源分配数据进行分析,得出了模糊需求可信性测度为0.833 3及最优分配方案的资源总支出为618 790元,明显低于实际的总成本为1 877 400元。研究表明文中所提出的优化模型通过对不确定性需求的有效估计得出的最佳资源优化方案可以为实际应用提供指导。
Faced with the increasing medical cost and limited medical service resources,the single-capacity medical service resource allocation is discussed in this paper. Based on diagnosis related groups( DRGs) and fuzzy demands,a model for systematic optimization is proposed. By this model,the uncertain demands are processed by using a fuzzy technique based on the credibility theory. With this model,a case for simple appendicitis surgery is analyzed based on a well-defined case type. By comparing the optimized total cost with the actual one,the model is shown to be able to optimize the allocation of medical resources with the fuzzy demand expectation with a 0. 833 3 certain credibility. The model is demonstrated to be effective and provides guidance for practical applications.
