钙化类型及分布与甲状腺癌的相关性分析 被引量:4

摘要 目的:探讨甲状腺结节中的钙化类型及分布特点与甲状腺癌的关系。方法:回顾性分析经手术病理证实的720例甲状腺结节患者(776个结节)的超声图像,依钙化大小及形状分为微钙化、粗钙化、周边钙化、钙化斑,分别统计其对良恶性的鉴别价值;将钙化分布类型分为中央型、边缘型和混合型,统计其与结节良恶性的关系。结果:776个结节中,存在钙化共319个(良性102个,恶性217个)。微钙化结节恶性发生率最高(78.9%),粗钙化、周边钙化及钙化斑亦有恶性的可能(发生率分别为42.9%、30.0%、40.0%);钙化分布方面,边缘型钙化多见于良性结节(70.6%),中央型、混合型钙化多见于恶性结节,尤其以中央型恶性比例最高(90.5%)。结论:任何形式的钙化都是恶性的风险因素,以微钙化特异性最高;钙化分布方面,中央型钙化恶性风险更高。
