原发性低颅压综合征的MRI诊断价值

The Diagnostic Value of MRI for Spontaneous Intracranial Hypotension Syndrome

目的探讨原发性低颅压(SIH)综合征的MRI诊断价值。方法回顾性分析11例SIH综合征的MRI表现,11例均行MRI平扫及增强扫描。结果 11例MRI增强扫描硬脑(脊)膜增厚并明显强化,9例上矢状窦、直窦、横窦扩张,7例脑沟、脑池变浅、双侧侧脑室变窄,6例双侧额颞部硬膜下积液,5例垂体饱满,1例小脑扁桃体下移,1例硬膜下少量出血,1例蛛网膜下腔出血。结论 SIH综合征的MRI表现具有特征性,结合临床可做出明确诊断。

Objective To evaluate MRI scanning in diagnosing spontaneous intracranial hypotension syndrome. Methods A total of 11 patients with spontaneous intracranial hypotension syndrome were enrolled in this study. Both plain and enhanced MRI scannings were performed in all patients. The MRI findings were retrospective analyzed. Results Contrast-enhanced MRI scanning showed that diffuse thickening of pachymeninges with obvious enhancement could be seen in all 11 patients. Expansion of superior sagittal sinus, straight sinus and transverse sinus was found in 9 patients, shallow sulcus, cistern and narrowed bilateral ventricles could be seen in 7 patients, and frontal and temple subdural collection of fluid was detected in 6 patients. Other findings included plumpness of pituitary body （ n = 5 ）, downward displacement of the cerebellar tonsil （n = 1 ） , subdural hemorrhage （n = 1 ） and subarachnoid hemorrhage （n = 1 ）. Conclusion Sponta- neous intracranial hypotension syndrome carries characteristic MRI features, based on which the diagnosis can be easily made especially when clinical information is taken into consideration.
