集束化护理对永久起搏器植入患者的影响分析

The influence of the bunlding nursing to the patients who have implanted the Heart PPM

目的探讨对永久性心脏起搏器植入患者实施集束化护理干预,以减轻术后出现的各种不适症状。方法选择2012年5月—2013年8月入住心血管内科,因心脏起搏和传导功能障碍需植入心脏起搏器的患者48例,随机分为试验组和对照组各24例。对照组运用护理程序采用常规护理,试验组在常规护理的基础上实施集束化护理干预,2组患者均采用Zung焦虑自评量表(SAS)和Zung抑郁自评量表(SDS)及数字分级法(NRS)进行心理状态及舒适情况评定。结果试验组患者接受集束化护理干预后焦虑评分(46.45±2.64)分,抑郁评分(50.12±3.35)分,术后腰背酸痛评分(4.82±0.95)分,切口疼痛评分(4.76±1.02)分;对照组评分分别为(59.42±1.68)分、(61.65±4.47)分、(5.91±1.25)分、(5.82±0.67)分。2组比较差异有统计学意义(t值分别20.27,18.95,3.52,3.31;P<0.01)。结论实施集束化护理干预可明显改善永久起搏器植入患者术后的焦虑和抑郁状态,提高患者的舒适度和生活质量。

Objective To investgate the bunlding nursing to the patients who have implanted the heart PPM inorder to reduce the unwell symptom. Methods To choose the 48 patiens in hospital who have heart disease with implanted the heart PPM during May. 2012 Aug. 2013 group them to test group and compared group. The test group used the normal nursing,while the compared group used the normal and bunlding nursing. To evaluate their mentation and comfort with SAS,SDS and NRS. Results Score the test group used the normal nursing,dysphoria： 46. 45 ± 2. 64,despondent： 50. 12 ± 3. 35, pain of the back： 4. 82 ± 0. 95,pain of the cut： 4. 76 ± 1. 02,the scores of the compared group： dysphoria（ 59. 42 ± 1. 68）,despondent（ 61. 65 ± 4. 47）,pain of the back（ 5. 91 ± 1. 25）,pain of the cut（ 5. 82 ± 0. 67） .（ t = 20. 27,18. 95, 3. 52,3. 31; P〈0. 01）. Conclusion To use the bunlding nursing can improve the dysphoria and despondent of the patients who have implanted the heart PPM,it can advance the comfort and life quality for the patients.
