患者自评-主观全面评定量表在胃肠恶性肿瘤患者营养筛查中的应用调查 被引量:50

摘要 目的探讨患者自评-主观全面评定(PG-SGA)量表作为胃肠恶性肿瘤患者营养筛查工具的有效性。方法采用便利抽样法,选取2013年2—8月在天津医科大学总医院普通外科行择期手术的住院胃肠恶性肿瘤患者100例。使用PG-SGA量表评估其营养状况,将其分为营养良好〔PG-SGA(A)〕组和营养不良〔PG-SGA(B+C)〕组。收集患者入院时的体质量、1个月内体质量下降百分比、体质指数(BMI)、血红蛋白(Hb)、清蛋白(Alb)、总淋巴细胞计数(TLC)等客观营养指标。结果经PG-SGA量表评定PG-SGA(A)组44例,PG-SGA(B+C)组56例,两组患者的性别构成、年龄比较差异均无统计学意义(χ2=0.807,t=-0.941,P>0.05)。PG-SGA(A)组的体质量、BMI、Hb、Alb、TLC均高于PG-SGA(B+C)组,而1个月内体质量下降百分比、PG-SGA总分均低于PG-SGA(B+C)组(P<0.05)。PG-SGA总分与体质量、BMI、Hb、Alb、TLC呈负相关(rs=-0.359、-0.439、-0.632、-0.442、-0.412,P<0.05),与1个月内体质量下降百分比呈正相关(rs=0.530,P<0.05)。结论 PG-SGA量表可很好地将胃肠恶性肿瘤营养良好与营养不良患者区分开,不但可以作为胃肠恶性肿瘤患者的营养筛查工具,而且为临床医护人员使用PG-SGA量表对胃肠恶性肿瘤患者进行营养筛查提供了参考依据。 Objective To investigate the application of the scored Patient-Generated Subjective Global Assessment(PG-SGA) in gastrointestinal cancer patients.Methods By convenience sampling method,100 gastrointestinal cancer patients from a general surgery department of Tianjin Medical University General Hospital were recruited from February to August,2013.Patients' nutritional status was evaluated by PG-SGA scale and according to the result was graded into good nutrition 〔PG-SGA(A) 〕group and under-nutrition 〔PG-SGA(B + C) 〕group.Objective nutritional indicators such as body weight,percentage of weight loss within one month,body mass index(BMI),hemoglobin(Hb),serum albumin(Alb),total lymphocyte count(TLC) were collected on admission.Results Patients in PG-SGA(A) group and PG-SGA(B + C) group by PG-SGA were 44 cases,56 respectively.There were no statistically differences in gender and age between the two groups(χ^2= 0.807,t =-0.941,P〈0.05).The objective nutritional indicators of PG-SGA(A) group in terms of body weight,BMI,Hb,Alb,TLC were higher than PG-SGA(B + C) group,while the percentage of weight loss within one month and PG-SGA scores were lower than PG-SGA(B + C) group(P〈0.05).PG-SGA scores were negatively correlated with body weight,BMI,Hb,Alb,TLC(rs=-0.359、-0.439、-0.632、-0.442、-0.412,P〈0.05).PG-SGA scores and the percentage of weight loss within one month were positively correlated(P〈0.05).Conclusion The PG-SGA scale can divide gastrointestinal cancer patients into well-nourished and malnourished.It can be used not only as a nutritional screening tool for gastrointestinal cancer patients,but also a reference for clinical medical staff.
