摘要 目的探讨非小细胞肺癌(NSCLC)患者外周血肺特异性X蛋白基因(LUNX)mRNA表达与血微转移的关系。方法选取90例NSCLC(实验组),其中Ⅰ期31例,Ⅱ期32例,ⅢA期27例;肺良性疾病组20例,分别于术前、术中关胸时、术后第7天抽取外周血,RT-PCR法检测LUNX mRNA表达情况,并与健康体检者10例对照。结果健康对照组及肺良性疾病组均无LUNX mRNA表达。实验组术前、术中、术后LUNX mRNA表达阳性率分别为46.67%、65.56%及20.00%,术中LUNX mRNA表达阳性率高于术前(P<0.01),术后低于术前(P<0.01)。Ⅰ期、Ⅱ期和ⅢA期患者术后第7天LUNX mRNA表达阳性率分别为6.45%、18.75%和37.04%,ⅢA期患者表达阳性率高于Ⅰ期、Ⅱ期(P<0.05)。结论 LUNX可作为NSCLC微转移较为敏感和特异的指标。检测肺癌患者LUNX是否可为肺癌患者制订更为个体化的治疗方案有待进一步研究。 Objective To explore the relationship between the expression of lung-specific X protein gene ( LUNX) mRNA and micrometastasis in the peripheral blood of patients with non-small cell lung cancer ( NSCLC ) . Methods Ninety patients with NSCLC ( experimental group ) included 31 cases of stageⅠ,32 cases of stageⅡand 27 cases of stageⅢA.Twenty cases of benign pulmonary disease ( benign pulmonary disease group ) and 10 healthy controls (control group) were enrolled into the study.The peripheral blood was collected from the subjects in the benign pulmonary disease group and control group before operation ,just after operation and 7 days after operation ,respectively ,the expression of LUNX mRNA in the peripheral blood was detected by RT-PCR,and a comparison of the results was conducted between two groups.Results None of LUNX mRNA was detected in the control group and benign pulmonary disease group .The positive rates of LUNX mRNA expression in the experimental group before operation ,during operation and 7 days after operation were 46.67%,65.56% and 20.00%,respectively,the positive rate of LUNX mRNA expression increased significantly during operation in contrast with that before operation ( P〈0 .01 ) , the positive rate of LUNX mRNA expression decreased significantly after operation in contrast with that before operation ( P〈0 .01 ) .The positive rates of LUNX mRNA expression 7 days after operation of patients with stage Ⅰ,Ⅱ and ⅢA NSCLC were 6.45%, 18.75%,37.04%,respectively,the positive rate of patients with stage ⅢA NSCLC was the highest ( P〈0.05). Conclusion LUNX might be a sensitive and specific marker for micrometastasis in NSCLC .Further study is needed to formulate a individual therapy for lung cancer based on the detection of LUNX in patients with lung cancer .
