膝关节半月板损伤MRI和关节镜诊断的比较研究

目的探讨MRI对膝关节半月板损伤的诊断准确率及其对临床治疗的指导意义。方法回顾性分析84例临床拟诊为半月板损伤的患者行关节镜术前的MRI检查资料,依关节镜诊断为金标准,将MRI诊断结果与关节镜诊断进行对比。结果 MRI诊断半月板损伤的灵敏度、特异度、准确度分别为84.8%,94.1%和90.5%。MRI和关节镜对半月板损伤具有极好的诊断一致性,Kappa值为0.87。结论 MRI对半月板损伤的诊断具有较高的准确度,对临床治疗方案的选择具有重要指导意义。

Objective To explore the accuracy of MRI in the diagnosis of meniscal injury and discuss its impact on clinical practice. Methods MRI was performed in 84 patients clinically diagnosis as meniscal injury before arthroscopy, according to the result of arthroscopy as golden standard, the results of MRI and arthroscopic diagnosis were compared. Results The sensitivity, specificity and accuracy of MRI for meniscal injury were 84.8%, 94.1% and 90.5%, MRI showed excellent Kappa agreements （0.87） for meniscal injuriy with the results of arthroscopy . Conclusion MRI shows a high accuracy in the diagnosis ofmeniscal injury and has important guiding significance to the clinical treatment options.
