常州市20年巨大儿发生趋势的动态分析

Dynamic Analysis of the Occurrence Trend of Macrosomia in Changzhou in Twenty Years

目的了解常州市巨大儿的发生情况,分析20年间巨大儿的发生趋势,为预防巨大儿的发生提供科学依据。方法回顾性分析1991-2010年20年间常州市妇幼保健院出生的活产儿临床资料,统计巨大儿数和分娩总数计算巨大儿的发生率。结果20年间常州市巨大儿发生率处于6.34%~9.27%,1991-1995年巨大儿的5年平均发生率为7.03%,1996-2000年巨大儿的5年平均发生率为7.97%,2001-2005年巨大儿的5年平均发生率为7.86%,2006-2010年的巨大儿的平均发生率为6.99%,（P〈0.01）。1991-2000年10年间巨大儿的发生呈明显的上升趋势（P〈0.01）,2001-2010年10年间巨大儿的发生略呈下降（ P=0.01）,2006-2010年较2001-2005年巨大儿的5年平均发生率较明显下降（ P〈0.01）。结论随着经济水平的增加,常州市的巨大儿发生率有上升的趋势,加强营养咨询和体重管理可有效地降低巨大儿的发生。

Objective To explore the occurrence of macrosomia in Changzhou, to analyze the trend of macrosomia in twenty years, and to provide scientific basis for preventing the occurrence of macrosomia. Methods A retrospective analysis of the clinical data of live births in Changzhou maternal and child health care hospital from 1991 to 2010 was made;and the incidence of macrosomia and total number of parturitions were statistically calculated. Results The incidence rate of macrosomia in Changzhou from 1991 to 2010 ranged from 6.34% to 9.27%, and the average incidence rate of macrosomia from 1991 to 1995 was 7.03%, the average inci-dence rate of macrosomia from 1996 to 2000 was 7.97%, during 2001 to 2005, this figure was 7.86%, while during 2006 to 2010, it went down to 6.99%. The incidence rate of the previous decade showed a clear upward trend, while the second decade showed a slight downward trend. Compared with that from 2001 to 2005, the average incidence rate of macrosomia from 2006 to 2010 decreased signifi-cantly. Conclusion There is a trend of increasing occurrence of macrosomia with the growth of economy. Improved nutrition counse-ling and weight management can effectively reduce the occurrence of macrosomia.
