Correlation between serum low-density lipoprotein levels and prognosis of patients with epithelial ovarian cancer

摘要 目的探讨血清低密度脂蛋白水平与上皮性卵巢癌患者预后的相关性。方法选取2010年5月~2013年10月就诊于我院的上皮性卵巢癌患者,分析血清总胆固醇(TC)、低密度脂蛋白(LDL)和甘油三酯(TG)水平与此类患者预后的相关性。结果研究共纳入33例患者,其中LDL升高的患者占30%,TC升高占18%,TG升高占32%,HDL升高占48%。升高的TC、HDL、TG、LDL与患者的年龄、疾病阶段之间无明显相关性。LDL水平升高患者的中位无进展生存期低于LDL正常的患者(12比27个月,P=0.0004)。同时,后者的整体生存期比前者长(59比51个月,P=0.04)。多因素分析表明,校正年龄、疾病阶段、分级和非理想肿瘤细胞减灭术后,LDL仍为患者远期生存的独立预测因素(P=0.003)。结论 LDL水平是上皮性卵巢癌患者远期临床预后的重要预测因素。 Objective To discuss correlation between serum low-density lipoprotein levels and prognosis of patients with epithelial ovarian cancer. Methods Identified patients with ovarian cancer and analyzed correlation between prognosis of patients with epithelial ovarian cancer and TC,LDL,TG. Results Selected 33 patients. 30% of patients had elevated LDL, 18% had elevated TC,32% had elevated TG and 48% had elevated HDL. No univariate associations were identified between elevated TC,HDL,TG,LDL and age,stage of disease,high grade or optimal cytoreduction. Median progression-free survival for patients with normal LDL levels was 27 months,compared with 12 months for patients with elevated LDL(P=0.0004). Overall disease-specific survival was longer for patients with normal LDL levels compared with those with elevated LDL(59 vs 51 months,P =0.04). Multivariate analysis indicated that LDL retained significance as an independent predictor of survival,after controlling age,stage,grade and suboptimal cytoreduction(P=0.003). Conclusion These data suggest LDL is a significant predictor for clinical outcome.
