直肠癌术后吻合口复发原因分析

Analysis of 18 Cases with Stomal Recurrence of Postoperative Rectal Cancer

目的 探讨和总结直肠癌术后复发的原因。方法 分析了近 10 a来收治的直肠癌术后吻合口复发患者 18例 ,未复发者 2 0例 ,对其病理类型、肿瘤体积及肿瘤远端切除长度进行比较。结果 肿瘤细胞分化差 ,体积大及肿瘤远侧切除长度短者易复发。结论 为减少吻合口复发 ,应根据肿瘤细胞的生物学行为、肿瘤体积大小及肿瘤距肛门的距离决定手术方式 ,根治是最重要的。

Objective To discuss and summarize the reasons for stomal recurrence of rectal carcinoma after operation.Methods 18 patients with stomal recurrence and 20 cases unrecurrent after operation were analyzed in recent 10 years.Their pathological types,the size of the tumors and the distance from tumor to inferior incisal edge were compared.Results The patients with poor differentiation carcinoma,large size of tumor and short distance from tumor to inferior incisal edge were prone to recurrence.Conclusions In order to reduce stomal recurrence,the selection of operative fashion should be determinated according to biobehavior,the size of the tumor the distance to anal edge.Radical resection is the most important approach.
