腹腔镜低位直肠癌保肛术后肠瘘原因分析及防治 被引量：2

摘要 目的 分析腹腔镜低位直肠癌保肛术后肠瘘的原因及进行有效预防、治疗的方法.方法 45例行腹腔镜低位直肠癌保肛术的患者中,术后发生肠瘘6例,对其临床资料予以回顾性分析.结果 5例患者经保守治疗瘘口自行闭合,痊愈出院.1例患者吻合口瘘发生后出现严重腹腔感染,行末段回肠造瘘、腹腔冲洗引流术,4个月后行造瘘口还纳治疗痊愈.结论 肠瘘的发生是多因素的结果,而低位直肠癌保肛术后由于其特殊的局部解剖关系,使肠瘘发生率更高.大多数低位肠瘘可通过保守治疗痊愈,而在术前或术中根据患者情况,果断地进行预防性回肠造瘘,可避免不必要的肠瘘发生.
