呼唤持续性心房颤动个体化消融治疗时代的到来

Calling the tailored approach to catheter ablation of persistent atrial fibrillation

摘要 心房颤动（房颤）的导管消融治疗已经成为全球各大电生理中心日常最主要的临床工作之一。这项工作现阶段亟需解决的难点之一是：如何在持续性房颤患者通过尽量少的消融损伤，实现较好的远期疗效。这实际上是一个关于如何实现持续性房颤导管消融治疗个体化的命题。毋庸置疑，不同持续性房颤患者的心房维持基质有显著差异，譬如，消融持续1个月的房颤和持续10年的房颤，二者在难度上很可能无法同日而语。几年前，由于持续性房颤的维持机制不明，对基质强弱亦缺乏可靠的评估手段，故只能施行固定术式的导管消融治疗。然而，随着一系列重要研究的发表，真正意义上的持续性房颤个体化消融时代可能已不再遥远。
