健脾补肾活血清热方联合西药治疗妊娠早中期宫腔血肿随机平行对照研究

Jianpi Bushen Huoxue Qingre Decoction Combined with Western Medicine in the Treatment of First and Second Trimester Uterine Hematoma Randomized Parallel Controlled Study

[目的]观察健脾补肾活血清热方联合西药治疗妊娠早中期宫腔血肿疗效。[方法]使用随机平行对照方法,将140例住院及门诊患者按抛硬币法分为两组。对照组62例根据检查结果适当采用HCG及黄体酮胶丸300mg/d,早100mg、晚200mg口服;或地屈孕酮予以黄体支持治疗。治疗组78例健脾补肾活血清热方(黄芪30g,白术炒15g,菟丝子20g,桑寄生15g,续断10g,当归6g,苎麻根20g,蒲公英、黄芩各10g,仙鹤草30g,升麻、陈皮、甘草炙各6g),1剂/d,水煎150mL,早晚口服;西医治疗同对照组。连续治疗14d为1疗程。观测临床症状、孕酮值、感染率、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组痊愈45例,有效21例,无效12例,总有效率84.62%。对照组痊愈20例,有效34例,无效8例,总有效率87.10%。治疗组疗效优于对照组(P<0.01)。孕酮值均有改善(P<0.05)。感染率治疗组少于对照组(P<0.05)。[结论]健脾补肾活血清热方联合西药治疗妊娠早中期宫腔血肿效果显著,值得推广。

[Objective] To observe the Jianpi Bushen Huoxue Decoction Combined with western medicine in the treatment of first and second trimester uterine hemorrhage effect. [Methods] Using randomized controlled Methods,140 cases of inpatients and outpatients by tossing a coin into two groups. A control group of 62 cases according to the inspection results to use the HCG and Progesterone Soft Capsules300mg/d,early 100mg,late 200mg oral Or dydrogesterone to luteal support. The treatment group of 78 cases of tonifying spleen and kidney and activating blood circulation and clearing heat（Huangqi 30g,Baizhu fried 15g,Tusizi 20g,Sangjisheng 15g,Xuduan 10g,Danggui 6g,Zhumageng 20g,Pugongying,Huangqin each 10g,Xianhecao 30g,Shengma,Chenpi,Gancao fried each 6g）,1 doses of /d,water simmer in water 150mL,sooner or later oral. Western medicine treatment with the control group. Continuous treatment 14d for 1 courses. Observation of clinical symptoms,progesterone,infection rate,adverse reaction,and the curative effect after continuous 2 courses of treatment. [Results] The treatment group cured 45 cases,effective 21 cases,invalid 12 cases,the total efficiency of 84.62%. The control group cured 20 cases,effective 34 cases,invalid 8 cases,the total efficiency of 87.10%. The treatment group was better than control group（P〈0.01）. Progesterone after course were improved in both groups（P〈0.05）. The infection rate in the treatment group was lower than in control group（P〈0.05）. [Conclusion] The effect that Jianpi Bushen Huoxue Qingre Decoction Combined with western medicine in the treatment of the first and second trimester uterine hematoma is significant,worthy of promotion.
