化痰活血方治疗痰瘀互阻型缺血性脑卒中随机平行对照研究

[目的]观察化痰活血方治疗痰瘀互阻型缺血性脑卒中疗效。[方法]使用随机平行对照方法,将60例住院患者按抽签法随机分为两组。对照组30例常规治疗,抗血小板聚集,控制血压、血糖、血脂等。允许使用脱水剂、抗生素等对症药物,禁止使用溶栓药物、降纤药物、其他抗凝药物;低盐低脂饮食,阿司匹林,100mg/次,1次/d;250mL吡拉西坦,1次/d,静注;疏血通疗法,维持水电解质平衡。治疗组30例化痰活血方(半夏9g,陈皮、当归各10g,地龙9g,三七粉10g,丹参30g,菖蒲、胆南星、桃仁、甘草各10g;形寒肢冷,大便稀溏,苔白滑,脉沉细加桂枝、干姜;身热,面赤潮红,大便干,舌红苔少,脉细数加熟地,山茱萸,栀子,酒大黄;头晕头痛、面赤、苔黄舌红、脉弦有力加天麻,钩藤,石决明,夏枯草;痰热偏盛加竹茹,贝母;咽干口燥加天冬,麦冬);1剂/d,水煎200mL,早晚温服;西药治疗同对照组。连续治疗14d为1疗程。观测临床症状、NIHSS评分、ADL评分、不良反应。治疗1疗程,判定疗效。[结果]治疗组显效8例,有效14例,无效8例,总有效率73.33%。对照组显效3例,有效7例,无效20例,总有效率66.67%。治疗组疗效优于对照组(P<0.05)。[结论]化痰活血方治疗痰瘀互阻型缺血性脑卒中效果显著,值得推广。
