患者状态指数在麻醉深度监测中的研究进展 被引量:5

摘要 传统的临床麻醉深度监测主要是通过观察术中患者的自主神经反射和躯体运动来判断,临床上常被用于调控麻醉和镇静深度的自主神经反射主要有:心率和血压、呼吸频率、呼吸形式、节律和潮气量、瞳孔对光反射、眼球运动、发汗及流泪、唾液分泌等,躯体运动反射主要是骨骼肌运动。但现代麻醉中,由于患者术前用药以及术中骨骼肌松弛药和血管活性药的联合使用,使得全麻状态下的临床体征难以十分准确客观地用来评估麻醉深度。比如大量血管活性或抑制性药物的应用,特别是降压药的使用,表现为心率、血压的下降,骨骼肌松弛药的使用,导致患者对疼痛刺激引起的躯体运动反射的消失以及其他药物的使用,阻滞腺体的分泌等,从而使得全身麻醉的麻醉深度、意识状态的改变常被掩盖或难以判断。
