低出生体重新生儿常用凝血功能参数变化的研究

Study on change of coagulation function parameters used in neonates with low birth weight

目的分析不同出生体重、不同胎龄的低出生体重儿的常用凝血功能参数变化及其临床意义。方法将187例低出生体重早产儿依据出生时的体重分为超低出生体重组、极低出生体生组和低出生体重组;依照胎龄不同分为<37周组、≥37周组;分析其凝血功能指标:凝血酶原时间(PT)、活化凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(Fib)的变化。结果不同出生体重组的PT、APTT及TT比较差异有统计学意义(P<0.05),而Fib比较差异无统计学意义(H=2.819,P>0.05);不同胎龄组的PT、APTT、TT、Fib比较差异均无统计学意义(P>0.05)。结论低出生体重儿的凝血功能指标(PT、APTT、TT)与其出生体重有关。体重越低的低体重新生儿更易发生凝血功能障碍。

Objective To change of coagulation function parameters used in different weight and gestational age of low birth weight infant and clinical significance. Methods According to the birth weight,187 low birth weight preterm infants were divided into the extremely low birth weight group,the very low birth weight and the low birth weight group. According to gestational age, low birth weight preterm infants were divided into the 〈37 weeks group and the ≥ 37 weeks group.Change of coagulation function indicators：prothrombin time （PT）,activated partial thromboplastin time （APTT）,thrombin time （Tr） and fibrinogen （Fib） were analyzed. Results There was a statistical difference of PT,AprIT and ＇IT in the different birth weight group （P〈0.05）,but there was no statistical difference of Fib in the different birth weight group （H=2.819 ,P〉0.05）;there was no statistical difference of PT, APTT, TT and Fib in the different gestational age group （P〉0.05）. Conclusion There is a relationship between coagulation function indicator （PT,APTT,TT） of low birth weight infant and their weight.In general,the lower weight,low birth weight infant are more likely to have coagulation dysfunction.
