摘要 目的 研究阿托伐他汀对冠状动脉慢血流患者血脂、炎症标志物、血管内皮功能的影响.方法 用校正的TIMI血流分级(CTFC)方法评价冠状动脉血流速度.于本院2012年8月~2013年10月因胸痛可疑冠心病行冠状动脉造影(CAG)检查的患者中选择诊断为慢血流的患者66例,随机分为他汀治疗组(31例)和常规治疗组(35例),他汀治疗组给予阿托伐他汀20 mg治疗4周,常规治疗组给予常规药物治疗.另选取36例有胸痛症状但CAG正常的无心脏病患者为正常对照组.采用ELISA法测定内皮素-1(ET-1)及白细胞介素6(IL-6)浓度,采用比色法测定NO水平.结果 他汀治疗组治疗后总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、三酰甘油(TG)水平显著降低,与治疗前比较差异有统计学意义(P<0.05).他汀治疗组治疗后ET-1、IL-6水平明显降低,NO水平明显升高,与治疗前比较差异有统计学意义(P<0.05).结论 短期服用阿托伐他汀治疗能改善冠状动脉慢血流患者的血管内皮功能. Objective To observe the effect of atorvastatin on lipid parameter,inflammation markers and endothelial function in patients with coronary slow flow.Methods The corrected TIMI frame count (CTFC) was used to evaluate coronary flow rates.66 cases of patients with coronary slow flow by underwent coronary angiography (CAG) because of chest pain from August 2012 to October 2013 were selected,and were randomly divided into atorvastatin treatment group (31 cases) and conventional therapy group (35 cases).The atorvastatin treatment group were received atorvastatin 20 mg for 4 weeks,the conventional therapy group were received conventional therapy.Another 36 cases of patients were selected as normal control group who suffered chest pain but the results of CAG came out normal.Serum ET-1 and IL-6 level were measured by ELISA.Serum NO was detected by chromatometry.Results After treatment,the TC,LDL-C,TG level of atorvastatin treatment group were significantly reduced,compared with before treatment,the difference was significant (P〈O.05).The NO,ET-I,IL-6 levels of atorvastatin treatment group were significantly improved,compared with before treatment,the difference was significant (P〈0.05).Condusion Taking short-term atorvastatin can improve coronary endothelial function in patients with coronary slow flow.
