妊娠期糖尿病产妇围生结局与血糖控制情况的关系研究

Relationship research of perinatal outcome of gestational diabetes mellitus and status of blood glucose control

目的 分析血糖控制与妊娠期糖尿病（GDM）产妇围生期结局之间的关系.方法 选取本院2012年1月～2013年6月收治的60例妊娠期糖尿病孕妇为研究对象,依照入院顺序随机分为观察组和对照组,对照组早晚餐前混合胰岛素皮下注射,观察组三餐前及睡前分别皮下注射短效和长效胰岛素,参照30例健康孕妇的相关资料,对比两组血糖控制效果及围生结局.结果 观察组血糖控制效果更佳、新生儿体重优于对照组（P＜0.05）,巨大儿比率与对照组差异无统计学意义（P＞0.05）;观察组均顺利分娩,围生结局优于对照组（P＜0.05）.结论 三餐前及睡前分别皮下注射短效和长效胰岛素血糖控制效果更佳,同时产妇围生结局更理想,此种治疗方案可作为妊娠期糖尿病的首选血糖控制途径.

Objective To analyze the relationship between blood glucose control and perinatal outcome of gestational diabetes mellitus （GDM）. Methods A total of 60 pregnant women with GDM admitted to our hospital from January 2012 to June 2013 were selected as the subjects,who were randomized divided into observation group and control group ac- cording to the sequence of admission.For the control group,mixed insulin was injected subcutaneously before breakfast and supper.For the observation group,short-acting insulin and long-acting insulin were injected before three meals and before sleeping,respectively.The blood glucose control effect and perinatal outcome of two groups were compared refer- ring to relative information of 30 healthy pregnant women. Results The blood glucose control effect and the neonatal weight of the observation group were better than those of the control group （P〈0.O5）,while the proportion of maerosomia showed no difference between the observation group and the control group （P〉O.05）.The cases of the observation group all accomplished successful labor,and the perinatal outcome was better than that of the control group （P〈0.05）. Con- elusion Subcutaneous injection of short-acting insulin and long-acting insulin before three meals and before sleeping respectively can accomplish better blood glucose control effect and more ideal perinatal outcome.Such regimen can be adopted as the preferred blood glucose control approach for GDM.
