摘要 目的研究凋亡抑制因子survivin在腮腺良恶性肿瘤中的表达及意义,以探讨腮腺肿瘤尤其是腮腺恶性肿瘤的发生、发展机制。方法用免疫组织化学法检测40例正常腮腺腺体组织、64例腮腺良性肿瘤及118例腮腺恶性肿瘤石蜡标本中survivin的表达,分析survivin的表达与腮腺肿瘤临床病理特征之间的关系。结果 survivin蛋白在正常腮腺腺体组织中不表达,腮腺良性肿瘤的阳性率为23.44%,恶性肿瘤的阳性率为64.41%,良恶性肿瘤间的表达差异有统计学意义(P<0.05)。腮腺恶性肿瘤中survivin的表达与肿瘤的恶性程度、TNM分期及肿瘤直径有关(P<0.05),但与患者年龄、性别、淋巴结转移情况之间尚不能明确其相关关系。结论 survivin的表达参与了腮腺肿瘤的发展变化,临床上可作为腮腺肿瘤诊断、治疗和预后的参考依据,也可作为日后肿瘤基因靶向治疗的重要参考。 Objective Survivin, an inhibitor of apoptosis has shown to be a significant prognostic indicator in various human canc- ers. The aim of this study was to investigate the expression and significance of survivin in parotid gland tumors, and then to explore the mechanism in the development of the disease. Methods We analyzed the survivin expression in 40 normal parotid gland tissue,64 benign parotid gland tumors and 118 malignant tumors of various histopathologic subtypes by means of immunohistochemistry. Results 23.44% (15/64) of benign tumors showed survivin expression, while 64.41% (76/118) of malignant tumors showed survivin expression. Normal parotid tissue was immunonegative. The expression of survivin was statistically significant between benign tumors and malignant tumors( P 〈 0.05 ). And the expression of survivin in malignant tumors had correlation with clinical stage, tumor size and the biological behavior of tumor(P 〈 0.05 ) , but with age, sex and lymph node status, their relationship is not yet clear( P 〉 0.05 ). Conclusion The expression of survivin was involved in the development and changes of parotid tumor. And it presents valuable reference in diagnosis, treatment and prognosis of parotid tumor. In the future it may be used as important reference gene in targeting therapy of cancer.
