摘要
目的评价18F标记的脱氧葡萄糖正电子发射计算机体层成像(18 F-FDG PET/CT)在结节病患者病情评估中的价值。方法回顾性分析15例不同累及范围及不同分期的结节病患者检查资料,比较CT和18F-FDG PET/CT对病变检出能力,分析最大标准摄取值(SUVmax)与病变累及区域数目及辅助检查指标的相关性,比较胸内结节病与全身结节病及不同分期的胸内结节病的SUVmax。结果 15例患者PET图像上均有纵隔和肺门淋巴结放射性浓聚,全身放射性浓聚最高部位SUVmax为2.3-18.9(6.83±4.31)。PET/CT检出37处病变区域,平扫CT检测出26处。SUVmax与病变累及区域数目、血清血管紧张素转换酶(SACE)及24-h尿钙浓度呈正相关(r=0.56,P=0.029;r=0.757,P=0.018;r=0.694,P=0.026)。结论 18F-FDG PET/CT可用于评价结节病的累及范围和病情活动程度。
Objective To investigate the value of ^18fluorodeoxyglucose (^18F-FDG) PET/CT in the evaluation of patients with sarcoidosis. Methods Data of 15 patients with sareoidosis were retrospectively analyzed, which was examined by ^18F-FDG PET/CT and conventional CT. The numbers of lesion areas were calculated and compared. The correlation of maximal standardized uptake value(SUVmax) with the numbers of lesion areas and the laboratory indicators were analyzed. The SUVmax was compared between the results reported by ^18F-FDG PET/CT and conventional CT. Results PET/CT showed an accumulation of ^18F-FDG at bilateral hilar and mediastinal lymph nodes in all patients with a SUVmax of 2.3-18. 9 (6.83±4.31). The numbers of lesion areas detected by PET/CT were more than those by CT(37 vs. 26). The SUVmax was positively correlated with the numbers of lesion areas ( r= 0. 56, P〈0.05 ), serum angiotensin converting enzyme ( r= 0. 757, P〈0. 05 ) and urinary calcium(r=0. 694, P〈0. 05). Conclusion ^18 F-FDG PET/CT may provide more evidences than conventional CT for the evaluation of the range and severity of sarcoidosis.
