摘要 目的了解性病门诊患者人乳头瘤病毒(HPV)感染情况和病毒亚型分布情况,为当地高危人群HPV感染流行病学研究和诊疗提供参考依据。方法于2012年1月-12月用PCR-反向斑点杂交法对本中心性病门诊1 786例患者进行HPV基因分型检测。结果在1 786例受检者中,男性HPV阳性检出率为47.33%,女性HPV阳性检出率为35.16%;男性感染HPV以6和11亚型为主,阳性率分别为38.42%和22.32%;而女性以HPV6,16,52和53型为主,阳性率分别为19.18%,15.89%,20.55%和16.16%;且男、女均以单一型HPV感染为主,感染率分别为67.23%和55.34%;以上差异均有统计学意义(P均<0.05)。受检男性和女性感染高峰期分别为30~39岁和20~29岁。结论性病门诊男性和女性患者感染HPV主要基因亚型不同,均以单一亚型感染为主,男性常见低危HPV6和11型,女性HPV6,16,52和53型感染率最高。女性HPV感染高危患者罹患宫颈癌的风险增加,高危人群感染HPV年龄为20~40岁。 Objective To assess the prevalence of the human papillomavirus (HPV) infection and its genotype distribution among STD out patients at our hospital, providing a basis for epidemiological study, clinical diagnosis and treatment of HPV infection in local HPV high-risk populations. Methods PCR-reverse dot blot hybridization was used to analyze the HPV genetic subtype in 1 786 patients in STD clinic during 2012. Results Among 1 786 samples, the HPV positive rates were 47.33% in men and 35.16% in women. In the HPV infected samples, men were mainly infected with subtypes 6 (38.42%) and 11 (22.32%)while women were mainly infected with subtypes 6 ( 19.18% ), 16 ( 15.89% ), 52 (20.55%) and 53 ( 16.16% ). The rates of single HPV genetic subtype were higher in males than females (67.23% vs. 55.34% , P 〈0.05). The peak ages of infection for men ranged from 30 to 39 years old while for women, from 20 to 29 years old. Conclusion The major genetic subtypes of HPV infection were different between men and women. Men were mainly infected with subtypes 6 and 11 while women were mainly infected with subtypes 6, 16, 52 and 53. The majority of the high risk populations had a single subtype of HPV infection. Women had a higher risk of developing cervical cancer, and HPV infection occurs mainly in the high risk populations aged 20 to 40 years.
