改良小切口白内障手术150例临床观察

Clinical Observation of Improved Mini-incision Cataract Surgery(150 Cases Report)

目的分析改良小切口非超声乳化白内障手术联合人工晶体植人术治疗的老年白内障患者的临床疗效。方法我院2012年3月至2013年9月收治150例（213眼）老年白内障患者，接受小切口非超声乳化白内障囊外摘除联合人工晶体植入术，对其术后视力及并发症进行临床分析。结果术中发生后囊破裂致玻璃体脱出6眼（2．8％）；虹膜根部断离8眼（3．8％）。术后发生虹膜炎症反应19眼（8．9％）；一次性高眼压9眼（4．2％）；角膜内皮水肿10眼（4．7％）。术后3—12个月视力0．5以上者104眼，0．3～0．5者89眼，0．1—0．3者10眼，低于0．1者10眼。患者脱残率为95．31％。结论行小切口非超声乳化白内障囊外摘除联合人工晶体植入术疗效显著且适应证广，特别适合在基层医院推广应用。

Objective To analyze the clinical effects of improved small incision non-phacoemulsification cataract surgery combined with intraocular lens implantation for senile cataract patients. Methods The postoperative visual acuity and complications were collected in 150 cases with 203 eyes of senile cataract in our hospital from March 2012 to September 2013. All the cases were treated with improved small incision nonphacoemulsification cataract surgery combined with intraocular lens implantation. Results There was posterior capsular rupture followed by vitreous hernia in 6 eyes （2.8%）, iris root rupture of 8 eyes （3.8%） intraopeartively. And iris inflammation in 19 eyes （8.9%）, one-off high pressure in 9 eyes （4.2%）, corneal endothelial edema in 10 eyes （4.7%） was occurred after operation. According to the naked eye eyesight, there were 10 eyes（4.7% ） less than 0.1, 10 eyes （4.7%） between 0.1 and 0.3, 89 eyes between 0.3 and 0.5, and 104 eyes more than 0.5. The residual-free rate was 95.31%. Conclusions It is effective of the improved small incision non-phacoemulsification cataract surgery combined with intraocular lens implantation for senile cataract patients with wide indication, especially suitable for the application in primary hospitals.
