外周血血浆中甲胎蛋白、丙酮酸激酶M2和白蛋白mRNA检测在乙型肝炎肝硬化患者早期诊断中的价值

Diagnostic value of alphafetoprotein, pyruvate kinase M2 and albumin mRNA expression in peripheral blood in hepatitis B virus-related cirrhosis

目的:探究外周血血浆中甲胎蛋白(alpha-fetoprotein,AFP)、丙酮酸激酶M2(pyruvate kinase M2,PKM2)和白蛋白(albumin,ALB)mRNA检测在乙型肝炎肝硬化患者诊断及预后中的价值分析.方法:选择2011-01/2013-12瑞安市人民医院收治的乙型肝炎肝硬化患者共102例作为观察组,并选取102例体检的健康人群作为对照组,所有受试者采用实时定量PCR方法,对AFP、P K M2和A L B基因的检出率、表达差异和mRNA与临床相关性进行分析.结果:计算AFP mRNA、PKM2 mRNA和ALB mRNA的2-??Ct,同时对AFP、PKM2及ALB的表达与临床生化指标的相关性,得到ALB mRNA和γ-谷氨酰基转移酶和血清白蛋白存在相关性.对所有患者经过联合检测后,乙型肝炎肝硬化组联合检测阳性率的最高,较为敏感,差异有统计学意义(P<0.05).结论:外周血AFP mRNA、PKM2 mRNA和ALB mRNA对乙型肝炎肝硬化患者的早期诊断具有一定的诊断价值,值得临床参考.

AIM： To assess the diagnostic value of alphafetoprotein （AFP）, pyruvate kinase M2 （PKM2） and albumin （ALB） mRNA expression in the peripheral blood in hepatitis B virus （HBV）-related cirrhosis. METHODS： One hundred patients with HBV-related cirrhosis who were treated at our hospital from January 2011 to December 2013 （observation group） and 102 healthy volunteers （control group） were included in this study. Real-time quantitative PCR was used to detect the mRNA expression of AFP, PKM2 and ALB in the peripheral blood of the above subjects. RESULTS： The expression of ALB mRNA in the peripheral blood was significantly correlated with y-glutamyltransferase and serum albumin. Joint detection of the mRNA expression of AFP, PKM2 and ALB in the peripheral blood had a significantly higher rate of detection of cirrhosis （P 〈 0.05）. CONCLUSION： The mRNA expression of AFP, PKM2 and ALB in the peripheral blood has important diagnostic value in patients with cirrho sis.
