联合检测在小儿肺炎诊断中的意义

Diagnostic significance of joint detection of PCT,CRP and WBC in children with pneumonia

目的探讨联合检测降钙素原(PCT)、C-反应蛋白(CRP)、白细胞计数(WBC)包括中性粒细胞分类计数(NC)在小儿肺炎诊断中的实用价值。方法选取57例肺炎患儿(细菌性肺炎30例、病毒性肺炎27例)和31例健康对照儿童进行PCT、CRP、WBC和NC检测,并将三组数据进行统计分析。结果细菌性肺炎组PCT、CRP、WBC和NC结果与病毒性肺炎组、健康对照组进行比较均有统计学差异(P均<0.05)。病毒性肺炎组与健康对照组比较,PCT、CRP、NC均有统计学差异(P均<0.05),而WBC比较无统计学差异(P>0.05)。细菌性肺炎组四项指标阳性率明显高于病毒性肺炎组(P<0.01)。结论 PCT对细菌性与病毒性肺炎有早期鉴别诊断价值,具有较高的灵敏性和特异性。联合检测PCT、CRP、WBC、NC的变化有助于鉴别患儿病原体类型。

Objective To investigate the practical value of combined detection of procalcitonin （ PCT） , C-reactive protein （CRP）, white blood cell count （WBC）, and neutrophil count （NC） in the diagnosis of children with pneumonia. Methods 57 children with pneumonia, including 30 cases of bacterial pneumonia and 27 cases of viral pneumonia, and 31 healthy control children were selected in this study. They were all given PCT, CRP, WBC and NC detection. Results The results of PCT, CRP, WBC and NC in the bacteria pneumonia group was different from the viral pneumonia group and the control group （ P〈0. 05 ） . The results of PCT, CRP and NC in the viral pneumonia group were statistically different from those in the control group （ P〈0. 05 ） , but WBC had no statistical difference between the two groups （P〉0. 05）. The positive rate of four indicators in the bacteria pneumonia group was significantly higher than that in the viral pneumonia group. In the bacterial pneumonia group, the positive rate of PCT was significantly higher than CRP, WBC and NC. Conclusion PCT has early diagnostic value on bacterial and viral pneumonia, with high sensitivity and specificity. The combined detection of PCT, CRP, WBC and NC can help to identify pathogen type.
