小剂量氯胺酮复合右美托咪定在大面积烧伤换药中的应用 被引量:10

摘要 目的观察小剂量氯胺酮复合右美托咪定在烧伤重症监护病房中非插管患者大面积烧伤换药时的麻醉效果。方法将38例大面积烧伤患者分两组,换药时分别给予单纯氯胺酮(对照组)、氯胺酮联合右美托咪定(试验组)麻醉,监测患者的心率、血压、呼吸、脉搏氧饱和度、术后镇痛效果及不良反应。结果对照组心率、血压在给药前较给药后5min、换药结束后20min升高(P<0.05),试验组无明显变化;试验组在氯胺酮用量、术后VAS评分、神经精神症状及术后呕吐发生率等方面低于对照组(P<0.05)。结论小剂量氯胺酮复合右美托咪定应用于有自主呼吸的大面积烧伤患者的换药中,对患者的呼吸循环影响更小,术中、术后并发症少,镇痛镇静效果良好。
