本文整理支持与反对"HMO(健康维护组织)"的理由,支持者注重财务的管控来达到效果,从供给面增加医疗服务提供商的财务控制责任,不但可以有效降低医疗费用,并且可以根除以往做多赚多对病人非有利的医疗商业机制。在提升医疗质量及效率方面,支持者认为HMO之"民营化"及"多元化"精神,可以促进市场"自由竞争机制"的形成,进一步可以提升医疗质量及效率。但反对者认为"费用控制的无效",且他们较注重人(病患与医师)的问题,批评此种节制医疗费用的制度将会对民众及医师的医疗质量造成破坏。本文建议若欲推动HMO必须做好制度与法律规范,并配合医疗结构转型,否则将会浮现反对者所提出的弊病。

This paper discusses the reason why people support or object ＂Health Maintenance Organization（HMO）＂. Supporters focus on the responsibility of financial control of medical service suppliers. This can reduce medical expense effectively and eliminate the drawbacks that are harmful to the patients. Regarding the medical quality improvement and efficiency,supporters believe that the＂privatization＂and＂diversity＂of HMO can promote ＂market competition＂ so that it can further enhance the quality and efficiency of healthcare. But opponents argue that ＂cost control is invalid＂. They more focus on people＇s problems and critique that the financial control would damage the quality of medical care from patients＇ and doctors＇ perspectives. The paper suggests that the government who intends to promote HMO must reform the relevant institutional and legal norms,and restructure the medical system,otherwise it will emerge the problems that opponents discuss.
