大肠癌及其癌前组织中EGFR蛋白的表达及意义

Expression of EGFR protein and its clinical significance in colorectal carcinoma and precancerous tissues

目的探讨EGFR蛋白在大肠癌及其癌前组织中的表达及临床意义。方法应用免疫组织化学SP法检测115例大肠癌、36例大肠腺瘤、43例癌旁大肠黏膜中EGFR的蛋白表达,并分析其与大肠癌临床病理特征的关系。结果①大肠癌、大肠腺瘤与癌旁大肠黏膜组EGFR蛋白的阳性表达率分别为62.61%(72/115)、30.56%(11/36)和12.77%(6/47),3组间差异有统计学意义(P<0.05)。其中大肠癌浸润深度达黏膜下层和肌层、浆膜下层及侵透浆膜层组EGFR蛋白阳性表达率分别为36.84%(7/19)、67.09%(53/79)和70.59%(12/17),后两者明显高于前者(P<0.05)。结论 EGFR蛋白表达与大肠癌组织浸润深度有关,提示EGFR可作为判断大肠癌预后的重要生物学指标;大肠癌中EGFR表达率较高,可为分子靶向药物在大肠癌中的应用提供理论依据。

【Objective】 To investigate the expression of EGFR and its clinical significance in colorectal carcinoma and precancerous tissues. 【Methods】 Immunohistochemistry method was used to detect the protein expressiones of EGFR in 115 cases of colorectal cancer, 36 cases of colorectal adenoma and 43 cases of adjacent normal mucosa and its relationship with clinic-pathological features of colorectal cancer. 【Results】 In carcinoma group, adenoma group and adjacent mucosa group, the positive expression rates of EGFR were 62.61%（72 / 115）, 30.56%（11 / 36） and 12.77%（6 / 47） respectively, the difference was statistic meaning among the three groups（P〈0.05）; In carcinoma that infiltrate to submucosa and muscular layers, subserous layer and penetrating subserous layer, the positive expression rates of EGFR were 36.84%（7 / 19）, 67.09%（53 / 79） and 70.59%（12 / 17） respectively, the latter two was higher than the former（P〈0.05）. 【Conclusion】 The expression of EGFR protein has positive correlation to the depth of invasion, this findings may help to predict the prognosis of colorectal carcinoma; positive high expression rates of EGFR in colorectal carcinoma provides a theoretical basis for application of molecular targeted drugs.
