家庭单位下高血压前期社区护理干预效果比较

Impact of a community based intervension for prehypertension care of the family

目的：研究并分析家庭单位下高血压前期社区护理干预效果。方法：选择2012年1月到2012年6月社区内舒张压在80~90 mmhg、收缩压在120~130 mmhg范围的人群为研究对象，共40例。所有患者均自愿参与本研究，签订知情同意书，对于高血压前期患者定期组织管理培训课程，制定好饮食与运动指导方案，加强与患者家属的沟通，及时了解患者饮食、运动情况，根据患者与家属在生活中遇到的各类健康问题进行指导，统计干预前后患者生化指标变化情况、不良生活方式变化情况以及对高血压相关知识的知晓情况。结果：本研究入组的40例患者与54例家属都顺利的完成随访。结果显示，社区护理干预后患者收缩压与舒张压降低，运动次数与运动时间得到有效的提升，干预后患者不良生活方式也得到有效的改善，患者与家属对于高血压知晓情况也显著优于护理干预前，组间比较差异显著（ P﹤0.05）。结论：家庭单位下的高血压前期社区护理模式成效显著，值得在社区中进行进一步的推广和使用。同时，为了强化教育效果，社区护理人员必须加强学习，提升自己的专业技能水平与责任意识，掌握沟通的技巧，这样才能够有效的发挥出社区服务中心的作用。

Objective：To study and analyze the effect of the family care for prehypertension with community nursing intervention. Method：Selecting a total of 40 patients with diastolic blood pressure in 80～90mmhg,systolic blood pressure in 120～130 mmhg from Jan. 2012 to June,2012 within the community. All patients were voluntarily participated in the study,informed consent signed,in prehypertension patients regularly organize management training courses,a good diet and exercise guidance plan system,strengthen the communication with patients and their families,timely information of the patient′s diet,exercise,conducted according to various health problems for patients and families encountered in life,patients′biochemical changes,bad life style statistics before and after the intervention and awareness on hypertension related knowledge. Results：40 cases of the enrolled patients and 54 family members were followed up successfully. The results showed that,the community nursing intervention in patients with systolic and diastolic blood pressure lowering,exercise frequency and exercise time effectively improved after the intervention,patients with bad life style also improved,patients and family members for the hypertension awareness was significantly better than before the nursing intervention,the difference between the two groups was statistically significan（P﹤0. 05）. Conclusion：The effect of the family care for prehy-pertension with community nursing intervention was significant,worthy of promotion and further use in the community. At the same time,in order to strengthen the effectiveness of education,community nursing staff must strengthen learning,enhance the professional skills and the sense of responsibility,master the skill of communication to work more efficiently.
