按柠蒎联合孟鲁司特治疗慢性阻塞性肺疾病急性加重期62例效果观察

Clinical Efficacy of Eucalyptol-Limonene-Pinene Enteric Soft Capsules Combined with Montelukast in Treating Acute Exacerbation of Chronic Obstructive Pulmonary Disease

目的观察桉柠蒎联合盂鲁司特治疗慢性阻塞性肺疾病急性加重期(AECOPD)患者的疗效。方法将122例患者随机分为治疗组(62例)和对照组(60例)。对照组进行氧疗、依据病原菌及药物敏感性试验结果选择抗生素、口服多索茶碱、雾化吸入布地奈德等常规治疗,治疗组在常规治疗基础上加用桉柠蒎0.3 g口服、每目4次,孟鲁司特10 mg口服、每晚1次,疗程均为2周。比较两组治疗前及治疗后2周血气分析和吸入支气管舒张剂后肺功能测定的相关指标。结果治疗2周后,治疗组第1秒用力呼气容积(FEv.)/用力肺活量(FVC)、FEVl较治疗前显著好转(t=6.38,P<0,01;t=6.22,P<0.01),且显著优于对照组(t=4.85,P<0.01;t=5.83,P<0.01);治疗组氧分压(PaO_2)、二氧化碳分压(PaCO_2)较治疗前显著改善(t=6.38,P<0.01;t=4.02,P<0.05),与对照组比较,PaO_2明显增加,PaCO_2显著降低(t=5.12,P<0.01;t=3.95,P<0.05)。结论桉柠蒎联合孟鲁司特可明显改善慢性阻塞性肺疾病急性加重期患者的肺功能。

Objective To observe the clinical efficacy of Eucalyptol- Limonene- Pinene Enteric Soft Capsules combined with montelukast in the treatment of acute exacerbations of chronic obstructive pulmonary disease（AECOPD）. Methods 122 patients with AECOPD were randomly divided into the treatment group（62 cases） and the control group（60 cases）.The control group received the conventional treatment of oxygen therapy, selecting antibiotics according to the pathogenic bacteria and the drug sensitivity test results, oral doxofyline and budesonide aerosol inhalation, while on this basis the treatment group was added with oral Eucalyptol- Limonene- Pinene Enteric Soft Capsules 0. 3 g, 4 times daily, oral montelukast 10 mg, once per night, with the treatment course of 2 weeks. The blood gas analysis before and after 2- week treatment and the related indexes of the pulmonary function test after inhalation of bronchodilator were checked and compared between the two groups. Results After the 2-week treatment, the FEV1/FVC, FEV1 in the treatment group were significantly improved than those before treatment（ t = 6. 38, P 〈 0. 01; t = 6.22, P 〈 0.01 ）, moreover which were superior to those in the control group（ t =4. 85, P 〈 0. 01; t = 5.83, P 〈 0. 01）; PaO2 and PaCO2 after treatment in the treatment group was better than those be- fore treatment; compared with the control group, PaO2 was significantly increased and PaCO2 was significantly decreased（ t =5. 12, P〈 0.01; t = 3.95, P 〈 O. 05）. Conclusion Eucalyptol - Limonene - Pinene Enteric Soft Capsules combined with montelukast can obviously improve the pulmonary function in the patients with AECOPD.
